Case Management Services

Increased Access to Care:
Case Management services are delivered in the community or via telehealth, reducing transportation barriers and improving access for rural and underserved populations.
Individualized Skill Building:
Case Managers help clients develop practical life skills, including coping strategies, emotional regulation, communication, budgeting, medication management, and daily living skills.
Improved Symptom Management:
Services focus on helping clients understand their mental health conditions and apply strategies to reduce symptoms and prevent relapse.
Care Coordination and Advocacy:
Case Managers coordinate with medical providers, schools, courts, family members, and other support systems to ensure continuity of care and reduce fragmentation of services.
Crisis Prevention and Stabilization:
Regular contact and proactive support help identify early warning signs, decreasing emergency room visits, hospitalizations, and crisis episodes.
Support for Independent Living:
Services assist clients in maintaining housing, employment, education, and community integration.
Family and Caregiver Support:
Case Management can include education and guidance for family members to strengthen natural supports and improve overall treatment outcomes.
Flexible, Client-Centered Approach:
Services are tailored to each individual’s treatment plan, goals, and cultural needs, promoting recovery and long-term stability.
Medicaid-Reimbursable Service in Ohio:
Case Management is a recognized Ohio Medicaid behavioral health service, making it financially accessible for eligible individuals.
